Conduct in-home, in-facility, or telehealth assessments using the UAS-NY assessment tool.
Establish and maintain positive relationships with consumers, caregivers, and facility staff.
Regularly review medical documentation and report any concerns or conflicting information.
Enter assessment data into the UAS-NY online application and transmit as required.
Provide direct supervision to assigned nursing and clinical staff.
Perform clinical case reviews and consultations.
Provide education and clinical training to staff in area of expertise.

Current Registered Nurse (RN) license valid in the state of New York.
Bachelor's degree and 5 years of experience or equivalent combination of education and experience required.
Minimum of 5 years of clinical experience.
Minimum 1 year supervisory experience preferred.
2+ years RN experience in-home care.
Ability to travel locally and reliable form of transportation.
