Registered Nurse Assessor - Hybrid Remote

About the position
Responsibilities
- Conduct in-home, in-facility, or telehealth assessments using the UAS-NY assessment tool.
- Establish and maintain positive relationships with consumers, caregivers, and facility staff.
- Regularly review medical documentation and report any concerns or conflicting information.
- Enter assessment data into the UAS-NY online application and transmit as required.
- Provide direct supervision to assigned nursing and clinical staff.
- Perform clinical case reviews and consultations.
- Provide education and clinical training to staff in area of expertise.
Requirements
- Current Registered Nurse (RN) license valid in the state of New York.
- Bachelor's degree and 5 years of experience or equivalent combination of education and experience required.
- Minimum of 5 years of clinical experience.
- Minimum 1 year supervisory experience preferred.
- 2+ years RN experience in-home care.
- Ability to travel locally and reliable form of transportation.
